Early on, you look for absolutes \u2014 normal versus high, good versus bad. Over time, you learn that the truth lives in direction, slope, and timing. It\u2019s not whether a number is elevated. It\u2019s what it\u2019s doing <em>now<\/em>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Yesterday\u2019s CEA result came back at <strong>65.5 \u00b5g\/L<\/strong>.<br>My second infusion is today.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Those two facts matter together.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>The Shape of the Curve Matters More Than the Height<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Over the past several weeks, my CEA has moved like this:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Mid-November: <strong>58.7<\/strong><\/li>\n\n\n\n<li>Early December: <strong>61.6<\/strong><\/li>\n\n\n\n<li>Mid-December: <strong>64.3<\/strong><\/li>\n\n\n\n<li>Yesterday: <strong>65.5<\/strong><\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">On paper, that\u2019s still \u201chigh.\u201d It\u2019s still well above the normal reference range. But biologically, this is not an accelerating curve. The increases are <em>slowing<\/em>, not steepening.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The most recent rise \u2014 from 64.3 to 65.5 \u2014 is smaller than the previous jump. That matters. It suggests that whatever momentum the disease had going into treatment is no longer increasing at the same rate.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">CEA doesn\u2019t fall overnight. Especially not with targeted systemic therapy. What you look for first is <strong>loss of acceleration<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That appears to be happening.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Timing Is Everything<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The most important detail here is timing.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This latest CEA draw reflects biology <strong>before<\/strong> my second infusion. It captures the tail end of the first cycle \u2014 a period when:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The drug has already been introduced<\/li>\n\n\n\n<li>Tumour cells are beginning to respond or resist<\/li>\n\n\n\n<li>Inflammation from cell stress can temporarily elevate markers<\/li>\n\n\n\n<li>True biochemical response often lags behind clinical action<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">In other words, this number is not a verdict on treatment failure or success. It\u2019s a snapshot taken mid-adjustment.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">What matters is that the curve hasn\u2019t steepened.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Stability Is a Signal, Not a Cop-Out<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">In oncology, stability is often misunderstood. People hear \u201cstable\u201d and think \u201cnothing is happening.\u201d In reality, stability during early cycles of systemic therapy is often the <em>first<\/em> sign that something is happening.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Unchecked disease tends to accelerate.<br>Treated disease often flattens before it falls.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Right now, the data suggests flattening.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Not reversal.<br>Not remission.<br>But containment.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And containment is how you earn time for the deeper work of treatment to take hold.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Why This Is Encouraging \u2014 Carefully, Not Casually<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Encouraging does not mean celebratory. It means <em>directionally favorable<\/em>.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The rise is slowing<\/li>\n\n\n\n<li>There is no sudden spike<\/li>\n\n\n\n<li>The marker is behaving in a controlled way<\/li>\n\n\n\n<li>Treatment has not yet had enough time to show its full effect<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">If the number had jumped sharply \u2014 70s, 80s, or higher \u2014 that would tell a different story. It didn\u2019t.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This is not the kind of result that forces a pivot. It\u2019s the kind that allows a plan to continue.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>What Comes Next<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The real signal will emerge over the next one to two cycles. That\u2019s when you look for:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Flattening \u2192 plateau<\/li>\n\n\n\n<li>Plateau \u2192 decline<\/li>\n\n\n\n<li>Or, if the drug isn\u2019t working, renewed acceleration<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Right now, none of those endpoints have declared themselves. What <em>has<\/em> declared itself is restraint.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And in cancer, restraint is meaningful.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Where I Stand Today<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">I\u2019m walking into my second infusion not with certainty, but with something just as important: <strong>no evidence that the treatment is failing early<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The numbers are still high.<br>The work is still ahead.<br>But the curve is not running away.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That\u2019s enough to keep moving forward.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Today isn\u2019t about celebrating a win.<br>It\u2019s about recognizing that the ground under my feet hasn\u2019t shifted against me.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Cancer has a habit of teaching you to read numbers differently.
